Employees who fall sick or sustain injuries at the workplace are entitled to workers’ compensation insurance benefits. The New Mexico Workers’ Compensation Administration oversees the state’s workers’ compensation program (WCA). 

The WCA manages the program’s management, which includes benefit payments and claim processing. The employer must be informed immediately after an employee gets hurt or falls sick. 

Below are common claim forms that you will need with regard to NM workers comp insurance.

First Report of Injury or Illness Form

One of the most crucial forms an employer must complete when an employee is hurt or becomes ill on the job is the First Report of Injury or Illness form. This form must be submitted within three days after the incident to notify the WCA about the injury or illness. The form asks for information about the worker, the company, and the specifics of the disease or injury.

Employee’s Claim for Compensation Form

Another crucial document required by the WCA is the Employee’s Claim for Compensation form. The employee submits this form in request for NM workers comp insurance benefits. This form covers information about the worker, the company, and the specifics of the illness or injury. The form also discloses any medical care the employee has undertaken and any lost income resulting from the injury or disease.

Employer’s Report of Injury or Illness Form

This form is usually submitted within ten days of the incident and is used by the employer to notify the WCA about the injury or illness. However, other forms may be required depending on the details of the claim. For instance, an employee may need to present additional forms to be eligible for long-term benefits if an injury results in a permanent disability.

Medical Report of Treatment Form

The treating physician must complete this form which includes information about the injury or illness and the recommended treatment. It includes details about the employee’s medical history, the diagnosis, the recommended treatment plan, and any restrictions or limitations on the employee’s ability to work.

It is crucial to remember that the WCA has rigorous guidelines and deadlines for submitting applications and claims. The claim may be denied or delayed if the employer or the employee fails to meet the deadlines. It is also crucial for employers to contact the WCA to inquire about the necessary documents and the due dates.
